GPs refer more than three quarters of people who go on to receive a diagnosis of cancer after only one or two consultations, shows an audit of cancer diagnosis in England. The audit concludes that the number of consultations is a useful indicator of the time interval between a patient presenting with cancer symptoms and being referred to see a specialist.1

Organisations not individual people are to blame, says Francis

The widespread system failure at Mid Staffordshire NHS Trust makes the identification of individual people responsible a “futile exercise” because so many are accountable, said Robert Francis QC in his report into the public inquiry.1 Naming names would “risk perpetuating the illusion that removal of particular individuals is all that is necessary” and focusing on “blame will perpetuate the cycle of defensiveness, concealment, lessons not being identified, and further harm.”

Hospitals are “very bad places for old, frail people,” the chief executive of the NHS Commissioning Board, David Nicholson, believes. Hospitals’ focus on getting a diagnosis, referring the patient to the right place, and getting treatment is unsuited to a patient population, 40% of whom will have some form of dementia, he told the Independent newspaper in an interview.1
