Doctors accused of being “horrible” over government’s cancer awareness campaign | BMJ

Doctors’ negativity threatened to derail plans for a government anticancer campaign that is now achieving positive results, a health official has said.

James Brandon, head of marketing for Public Health England, said that many GPs and other health professionals were “horrible” towards the Be Clear on Cancer campaign when it was conceived in 2011 by the Department of Health.

Background

Self-harm is prevalent in adolescence. It is often a behaviour without verbal expression, seeking relief from a distressed state of mind. As most adolescents who self-harm do not seek help, the nature of adolescent self-harm and reasons for not disclosing it are a public health concern. This study aims to increase understanding about how adolescents in the community speak about self-harm; exploring their attitudes towards and experiences of disclosure and help-seeking. Pregnant women who request a caesarean section should discuss pros and cons with maternity team, says NICE in new quality standard

The National Institute for Health and Care Excellence (NICE) has published standards to improve the quality of care for women who may need or ask for a caesarean section.

The number of women having caesarean sections has gone up dramatically in the last thirty years from fewer than one in 10 (9%) in 1980 to up to a quarter of births (around 20-25%) in 2013i.

The NICE quality standard for caesarean section is designed to measure and improve the care of women who may need, request or have had this procedure. In summary:

  • The pregnant woman should have a documented discussion with the members of the maternity team about the risks and benefits of caesarean section compared with vaginal birth. She should also be able to talk to the most relevant member of the maternity team depending on her question or concern at any time during her pregnancy. The discussion should be arranged promptly. Anecdotal evidence suggests there is confusion among mothers-to-be surrounding caesarean sections. If a woman chooses to plan a vaginal birth after she has previously given birth by caesarean section, she should be fully supported in her choice.
  • Women who ask for a caesarean section because they are worried about childbirth should be able to discuss their concerns with a suitably trained healthcare professional (an expert in perinatal mental health support).
  • A consultant should be involved in decisions surrounding caesarean sections because they are best placed to advise about the potential benefits and risks. Their involvement should ensure the best possible outcomes for the mother and the baby.
  • The maternity team should offer to take a blood sample of a baby when an emergency caesarean section is being considered if the child is thought to be distressed or unwell. Taking a blood sample gives the mother and her maternity team more information when deciding whether to choose a caesarean section or to continue with a vaginal delivery.
  • After the caesarean section, a woman and her partner should be able to discuss the implications for future pregnancies so they are able to make more informed decisions about planning their family. The information should be given in a discussion and on paper.
  • Women who have had a caesarean section should be monitored for complications in the hours after the caesarean section surgery.

Abstract

Background Many overweight people take action to lose weight but most regain this weight.

Purpose To examine the effectiveness of a weight maintenance intervention focused on regular self-weighing after receiving a 12-week weight loss programme.

Methods Quasi-randomized controlled trial of 3768 obese or overweight men and women. The intervention group (n = 3290) received two telephone calls, the offer of free weighing scales, encouragement to weigh themselves weekly and record this on a card. The main outcome was change in weight between 3 and 12 months. Read more of this post
